Graduated Response

Tier 1

  • Structured conversation with parent/carer and child
  • Speak to others that know the child – previous teacher/s, parent,etc
  • Refer to reasonable adjustments in SEN File- see SENDCO
  • Pupil passport
  • Tracking document (to identify triggers/times/lesson/ teacher etc)
  • Raise concerns with SENDCO or KS manager
  • Observation by SENCO/Behaviour Lead (Behaviour for Learning audit and evidence adjustments made)
  • Provision Map (identifying relevant school based interventions)
  • Surgery with GAP team

 

Tier 2

  • Family Springboard-
  • Consultation with GAP Team( Consent for discussion required)
  •  2nd consultation to follow up on recommendations made (at this point it will be decided if a referral to GAP is appropriate or if the child and school can be supported within our consultation model)
  • Seek Family support
  • Specific 1:1 or small group interventions – see SENDCO
  • School nurse referral if appropriate
  • Apply for additional funding

 

 

Tier Three

  • Request for support to appropriate Support to Schools Team- GAP/EPS/SFSS
  • Paperwork for GAP referral can be obtained from gedling-area-partnership.secure-primarysite.net
  • CAMHS Consultation if appropriate
  • Referral to Small Steps Nottinghamshire
